 **Please remember to bring the following with you for your appointment:

• Driver’s license or another photo I.D.
• Social Security cards for yourself and all dependents
• Bank account information for direct deposit of any refund
• W-2 from each employer
• Documentation of all other income (1099R, Social Security Statements, etc.)
• Form 1095-A if you received health-care coverage through the marketplace
  • Unsure if you need this form? – call 1-800-318-2596 – select “tax” from options
• If you received overtime pay in 2025 – last pay period paystub for 2025
• IPPIN – An identity protection PIN (IPPIN) is a six-digit number that prevents someone else from filing a tax return using your Social Security number (SSN).
  • **If you have a pin already but do not have the letter - go to IRS.gov and log into online account to get PIN or call 1-800-908-4490

      *If filing a joint return, both spouses must be present to sign the tax return.
